The main objective of the Dorchester County School District Two Health Program is to assist each student in achieving and maintaining optimal health so that maximal physical, social, emotional, and academic growth can occur.
School Health Room Operations: The health office is designed to be an emergency station to care for minor illnesses or injuries that occur at school. Injuries or health concerns that occur outside of school should be treated at home or by your healthcare provider. The school nurse cannot diagnose or prescribe treatment. This is the responsibility of your family physician or health care provider.
School Exclusion List: WHEN SHOULD I KEEP MY CHILD HOME FROM SCHOOL? There are times when a student should remain at home for his/her own welfare and for the protection of others. Please refer to the DHEC School Exclusion List for specific information.
Spann Health Office Guidelines:
- Students must have a pass from their teacher to be seen. If a student wants to be seen in between class periods they should check in with their next period teacher first and get a pass.
- Our main goal is to optimize student health and enhance learning. To learn you must be present! If your child does not have a fever and is not throwing up they will not be sent home.
- Reasons we will send a child home: a temperature greater than 100 degrees, active vomiting or diarrhea, suspected contagious illness, a serious injury, or appears obviously ill.
- 24 HOUR RULE OF THUMB: A student should be without fever, nausea, vomiting, and diarrhea for 24 HOURS before returning to school.
- We are happy to help by providing band aids or ice but injuries that happen outside of school should be taken care of at home.
- Encourage your child to WASH THEIR HANDS! It is the #1 way to keep from getting sick.

Medication At School:
Parents must register ALL prescription and over-the-counter medications with the school nurse. In order to ensure the safety and medical well-being of all students here at Spann, the following guidelines must be adhered to.
- Parent/legal guardian must complete and submit a Dorchester School District Two Medication Permission Form. This form requires both the signatures of the physician and the parent/legal guardian and the specific time(s) to be given.
- Parent/legal guardian must bring the medication in the current prescription bottle properly labeled by a registered pharmacist as prescribed by law (ask your pharmacist to prepare a separate labeled bottle for school use).
- Medication must be brought in by the parent or responsible adult, NOT THE STUDENT.
Parents are reminded that aspirin, cough medicine, vitamins, cough drops, all over-the-counter products, will not be given at school without a written prescription from a doctor stating the medication, dosage, and specific time(s) to be given at school. - All medication given at school (even over the counter) must have a prescription label. Please check with your physician about writing a prescription for over the counter medication that can be picked up at the pharmacy.
- Herbals, food supplements, alternative medicinal products, and other items that do not have FDA approval will not be given at school without a written prescription from a doctor stating the medication, dosage, and specific time(s) to be given at school.
- All medicine not registered with the school nurse will be in direct violation of district policy and dealt with accordingly.
- Parents are also reminded that school personnel will dispose of medication not claimed at the end of the school year. All medication will be handled in accordance with the above guidelines through the school nurse or principal’s designee.
- The first dose of medication that a student has not taken before should be given by the parent/guardian at home so that the student can be monitored closely for hypersensitivity and side effects.
Official Medication Forms: These are the forms that are necessary for your child to have medication at school. These must be renewed every year.
1. Parent Authorization Form: This permits your child to carry medication on their person while at school. This only includes medication that are needed for emergency purposes such as an inhaler or epi-pen. This should be completed by you. Click here to download the form.
2. Health Care Practitioner Authorization Form: This permits your child to carry medication on their person while at school and must be signed by your child’s health care provider. Click here to download the Doctor Order Form. Click here to download the Student Self-Monitoring Form.
3. Medication Policy and Physician Order Form: This is to be signed by your child’s health care provider and means that we will keep medication in the nurse’s office for them to receive either on a daily basis or as needed. Click here to download Side 1. Click here to download Side 2.
Immunizations: A valid South Carolina Certificate of Immunization is required for every student attending school in South Carolina. “NO SHOTS, NO SCHOOL!” Doses documented on the immunization certificate must be valid doses according to accepted practice standards for the minimum age intervals. Your student’s physician and/or SC DHEC should be able to provide you with the required SC immunization certificate. If your child’s records are found to be incomplete you will be notified by the school nurse. You will then have 30 days to present a valid certificate or your child may be suspended from school until this is supplied by the parent or guardian.
DHEC Immunization Requirements for 2016-2017 School Year
Chronic Conditions Management: Some students have health needs that must be taken care of during the school day. Many health care services can be provided to students at school which will keep students at school where they can learn. If your child has a chronic condition such as diabetes, asthma, severe allergy, seizures, etc. please notify your school nurse immediately! This allows us to put in place any necessary emergency plans and also write an Individualized Health Care Plan which is mandated by the state of South Carolina.
Parent/Guardian IHP Declination Statement: It is within a parent’s right to decline a health plan for their child. In the event that you do not wish for your child to have one you must submit the declination form. Please be assured that your child will still receive the highest quality of care that falls within district policy and is based on best nursing practice.
