Student Health Information Card

Medication At School

Please register all medication with the nurse's office, including over the counter medication.

Immunizations

Parents will be notified by the school nurse if immunization records are missing or needed. Click here to view the DHEC Immunization Requirements. Students have 30 days from enrollment to submit shot records using DHEC Form 2740.
